Social Services
Heritage Hospice Medical Social Workers provide assistance with practical and financial concerns as well as emotional support and counseling for patients and their families.
They evaluate the need for volunteers and other support services needed by the family and facilitate communication between the family and community agencies. They also serve as a resource on issues related to death, dying, grief and bereavement.
Our social services are based on an initial visit to assess the psychosocial and social needs of the patient and family. It is provided in accordance with the plan of care to assist the patient and family in dealing with the psychosocial aspect of a life-limiting illness.
Our social workers are skilled in interviewing techniques and evaluation skills and have knowledge about available community resources. They offer individual and family counseling related to living with an end-stage illness.
