Heritage Hospice is here to help. We provide expert medical, emotional, spiritual, bereavement, and volunteer care for patients with any terminal illness and a life expectancy of six months or less. Plus we offer support to the entire family, enabling loved ones to spend more time together for a deeply fulfilling experience.

Heritage Hospice recognizes that a terminal illness presents an extremely difficult time in anyone’s life. Because we understand how overwhelming and challenging this time can be for the patient and their family, we offer free evaluations to assess the patient and see if hospice care is appropriate. At the evaluation, we will be happy to discuss the scope and nature of hospice care with both the patient and their family.
Referrals to the Heritage Hospice program should be made as early in the terminal phase as possible. Early referrals (months before death versus days before death) provide the patient and family more opportunity to work through the issues: grief, symptom management, financial, and personal arrangements. The final phase of life can be filled with quality and love, so death may be met with dignity.
Reimbursement sources for services include Medicare, Medicaid, Private Insurance, and Private Pay. We do not deny services based on inability to pay.
