Our therapists and assistants are licensed within the state of Indiana to provide a broad range of rehabilitation services. The rehabilitation team implements individualized patient treatment programs aimed at achieving specific patient and family goals for optimal levels of functioning.

We offer exclusive outpatient services as well as in house rehab services to fit your long term plans, whether your plans are to stay with us, progress to enhanced assisted living, or return home. Our services are available to residents of The Heritage Center and The Village at Holiday, as well as outpatients in need of our specialized services.
