Heritage Hospice promotes dignity and emphasizes quality of life for those who choose to die in familiar surroundings with those they love. Our services can be provided at home, a skilled nursing facility or an assisted living center.
Heritage Hospice provides personalized care directed at helping the patient and family handle the entire range of physical, emotional, social and spiritual needs when a loved one is dying. With a focus on comfort rather than curative care, Heritage Hospice emphasizes relief of physical symptoms and emotional support. Our care centers on: Pain and symptom control; Spiritual support as needed; Bereavement support for families and friends; and Emotional support.
We provide services, support and coordinate care for individuals who are diagnosed with a life-limiting illness with a prognosis of six months or less so that they might live as fully and comfortably as possible.
Heritage Hospice interdisciplinary team of professionals – physicians, nurses, medical social worker, spiritual care coordinator, home health aides, volunteer coordinator, and volunteers– work together to manage the individualized care plans and needs of each patient and family, enabling them to complete their lives in peace with a sense of grace, dignity and comfort.
