Administrative Staff
Dora Gaylor, SPHR, Director of Human ResourcesDora has been employed at The Heritage Center for over 23 years. As the Human Resource Director she is responsible for employee benefit administration, payroll administration, personnel relations, HIPPA Compliance, and serves as our 401-K Administrator and Security Officer. Working closely with staff, Dora maintains a balance of compassion and professionalism that make working for Holiday Health Care an enjoyable experience.
Susan Straub, R.D., C.D., Registered DietitianA graduate of Purdue University with a Bachelor of Science degree, Susan has been with the Heritage Center since 1985 as our Registered Dietitian. She oversees our food services department, supervising the selection, preparation, and serving of meals. Nutrition is a priority in working toward the overall health of each resident.
Kathy Hillyard, Business Office ManagerWith all the challenges and changes in Medicare, Medicaid, and other insurance providers, it is imperative for a healthcare facility to have a business office manager who is both knowledgeable about finances and sensitive to the needs of residents and families. Kathy oversees all accounts for current residents as well as assisting new residents and their families in working through the financial and transitional issues that come with utilizing a healthcare facility. She regularly attends accredited Medicare training seminars. Her goal is for families and residents to be comfortable and secure when working through financial issues in the business office.
Lisa Beyers, Director, Horizons Rehabilitation CenterLisa has worked in long term care for eighteen years. She has owned and managed therapy contract services and outpatient clinics since 1993. In 1998 she joined forces with Progressive Health Rehabilitation and served as the Vice President of contract services until 2003 when she began consulting as a rehab specialist and licensed health facility administrator. Lisa currently manages our in-house therapy department. “This is the most diverse and talented team of therapists I have ever worked with."
As Admission Director, Jayme has the opportunity to assist residents, patients, and their families in the process of utilizing the services of the Heritage Center and Horizons Rehabilitation Center. For many families, this process may be unfamiliar. Jayme's goal is to walk each family through the process, arranging for the best possible care and accommodations to help residents and patients achieve their optimum level of recovery and quality of life.
Linda Koressel, LPN, SSD, AdmissionsLinda has been at The Heritage Center since 1986. She works closely with families, residents, and community contacts to find the most appropriate care for residents based on their needs and preferences. Her duties not only include initial admissions, but ongoing assessment and interaction with residents to meet their current needs. Linda also oversees bed utilization within the facility.
Megan serves as Social Service Director at the Heritage Center. She graduated from the University of Southern Indiana, completing a Bachelor's degree in 2007, and a Master's degree in Social Work in 2010. The most rewarding part of her job is meeting new people and having the opportunity to learn more about them. She oversees the social service needs on the Hearthside Unit and serves as program director for the Reflections Unit. Megan works closely with staff and families who are caring for loved ones affected by Alzheimer's Disease and/or other dementia- related illnesses. She has special training in the Best Friends approach to dementia care and is pro-active in Alzheimer's Association events. The monthly Alzheimer's Support Group Meeting is facilitated by Megan.
Fran has been with the Heritage Center at Holiday since 2003 and holds a Social Service Designee certificate. Experienced with many of the social needs associated with long term care, she is a committed advocate for residents on the Harbor Unit. She works closely with residents, their families, and staff to provide a comfortable and satisfactory living environment for residents.
Kenya oversees the social service needs of residents on the Harbor Unit. She is resident-focused, looking out for the needs and preferences of those who reside on the Harbor Unit.
Sandy Taylor, AD, Bridges DirectorSandy has worked for The Heritage Center for 9 years, primarily in the area of Spiritual Care before assuming the position of Bridges Director. As Director of this diverse department, she oversees the planning and implementation of all activities for residents, Spiritual Care for residents, families, and staff, and Heritage Volunteers who provide friendship, assistance, and community interaction for the residents.
The Companion Program and Transportation Services are also part of the Bridges Department.
Jean is committed to providing a clean, safe environment for the residents and patients of the Heritage Center and Horizons Rehabilitation Center.
David Bretz, Maintenance DirectorDavid has worked for The Heritage Center since 2000. Dave is responsible for maintaining equipment, lawn/ garden care, environmental safety and subcontracted maintenance labor. David oversees a staff which cares for the entire Holiday Health Care campus, including the Heritage Center, Physicians and Diagnostic Center, and Holiday Retirement Village. He also serves as Assistant Fire Chief for the German Township Fire Department where he is highly trained and experienced in all aspects of disaster preparedness and response.
Katie Inkenbrandt, Food Services Manager
Kati started working for Holiday Health Care at The Village at the age of 15. Currently attending IVY Tech Community College, she is pursuing an Associate’s Degree in Hospitality Administration and Restaurant Management. She is certified in First Aid, CPR, and Serve Safe Food Safety. Kati has a passion for providing an enjoyable and enticing fine dining experience for our residents and strives to provide the highest quality service available. She maintains wonderful relationships with all of the residents, getting to know their needs on a professional and personal basis.
Nursing Administrative Staff
Joanne has over 15 years experience in Nursing Administration. In her role as Director of Nursing, she is responsible for all aspects of nursing care throughout the facility and is instrumental in creating an environment that provides quality care and a secure environment for our residents.
Cindy Elmore, LPN, Assistant Director of NursingCompliance Director
Cindy is the Assistant Director of Nursing and Compliance Director with over 36 years of service with The Heritage Center. She is familiar with every unit in our facility and assesses each resident prior to admission for appropriate nursing/unit placement and care. She also coordinates physicians visits and oversees compliance.
Marina Tieken, LPN, Director of Personnel ServicesMarina directs the application process, interviewing, hiring, training and orientation of all staff. Once hired, she oversees in-service education, along with the administration of personnel policies and procedures. Marina serves as an instructor for the following: Certified Nursing Assistant, Dining Assistant, TB, and AHA BLS. She also handles certificate programs for our licensed professionals.
Her goal "…to see others become all they can be."
Michelle Williams, LPN, Director of Quality CareMichelle has worked in many capacities in the nursing field in her 21 years of service at The Heritage Center. Michelle is not only the Director of Quality Care, but also serves as our liaison for Hospice Care and supervises the Restorative Nursing Program.
Mary Lynn Roberts, RN, Unit DirectorMary Lynn is the Unit Director for Horizons Rehabilitation Center and Reflections Unit, overseeing all aspects of department management. The Horizons Rehabilitation Center is designed to work with patients who are utilizing therapy and rehabilitation services. Reflections is a secure unit which serves residents with Alzheimer's and other dementia-related disorders.
Kim Braukhoff, LPN, Unit DirectorAs Unit Director, Kim oversees the care of 60 intermedial care residents on Harbor, along with the Hearthside Unit. Hearthside is a 19-bed unit which accommodates the needs of those in progressive assisted living. She is committed to making a difference in the care of our residents and loves to see their smiles. Kim is a graduate of Evansville School of Practical Nursing (1981) and has 33 years experience in Geriatric nursing.
Jennifer Phillips, LPN, Infection Control and Prevention Team NurseJennifer has had extensive training in wound care, graduating from Ivy Tech in 1992. She monitors wound and infection control issues on the Horizons and Reflections Units. "I love the interaction with the residents... hearing all of the stories of their different lives, remembering that they are individuals who depend on us to provide the best care possible." Jennifer is a member of Indiana's Health Care Associated Infection Initiative.
Jane Brown, RN, Evening SupervisorEvening shift Supervisor for Harbor and Hearthside. Jane worked in long term care as a CNA before graduating in 1979 from Wabash Valley College as an LPN. As a 1990 graduate of Frontier Community College in Fairfield, IL, Jane has been MDS Coordinator for an 80 bed facility, Staff Development Coordinator in a 100 bed facility, and an RN working the floor as a Charge Nurse. She enjoys working closely with the staff she supervises as they serve to meet the needs of our residents.
Phyllis Nussmeyer, RN, Night and Weekend SupervisorPhyllis is a graduate of Deaconess School of Nursing and oversees our nursing staff nights and weekends. She enjoys assisting staff with their questions and education, taking care of residents, and working with resident families. Phyllis lives out her personal mission statement, “Do unto others as you would have them do unto you.”
